Your monitoring dashboard is gorgeous. Average API response time: 48ms. Error rate: 0.1%. SLOs: green across the board. Your PM sends a thumbs up in Slack.
Then you open the support queue. "App felt really slow today." "Had to reload three times." "Is the site down?"
You're not gaslighting yourself. Your users aren't wrong. Your average is lying.
This is the tail latency problem — and if you're only watching averages, you're flying blind over the exact terrain where users suffer most.
The Average Is a Bad Liar
Picture 100 simultaneous requests hitting your /api/dashboard endpoint. Ninety-five respond in 30ms. Five of them — the unlucky ones — hit a GC pause, a locked database row, or a saturated connection pool, and wait 800ms.
Do the math:
Average = (95 × 30ms + 5 × 800ms) / 100 = 68.5ms
Dashboard: healthy. Reality: 5% of users just had a rough time. If your service handles 10,000 requests per minute, that's 500 users per minute experiencing an 800ms delay. Over a day that's 720,000 bad experiences, all invisible in your "healthy" average metric.
P95 would surface this. P99 would scream about it. The average silently buried it.
The "tail" in tail latency refers to the right side of your latency distribution histogram — the slow outliers that drag on user experience without meaningfully moving the mean. The tail is where real performance problems live, and it's where your angriest users come from.
What Actually Creates the Tail
The frustrating part is that tail latency causes tend to be intermittent and hard to reproduce locally. Here's the usual lineup:
GC pauses — Node.js (V8) performs periodic major garbage collection. A major GC can pause your event loop for 50–200ms. It happens infrequently, which means it barely touches the average but reliably inflates P99.
Lock contention — A database row-level lock held by a long transaction. 99% of requests never touch that row. The 1% that do queue and wait. Your average stays clean; your P99 takes the hit.
Connection pool exhaustion — Pool at capacity, requests queue for an open slot. The first requests through are fine. The last ones wait. Undersizing your pool is one of the most common P99 culprits I've seen — we caught this on a high-traffic API at Cubet where P99 was creeping up while P50 barely moved. Pool size was the smoking gun.
Noisy neighbours — In any cloud environment, your VMs share CPU cycles and network bandwidth with other tenants. Most of the time it's invisible. Occasionally another tenant spikes and you feel a jitter that shows up exactly in your tail.
Cold code paths — Error handlers, cache miss fallbacks, retry logic — all the code paths that rarely execute. Because they're rare, nobody optimises them. When they do fire, they're slow. And they show up in P99.
Measuring Tail Latency in Your App
You need percentile-aware instrumentation, not averages. Here's a practical starting point for an Express service:

This is enough to get you started. For production-grade work, reach for HdrHistogram — it's memory-efficient, designed exactly for this use case, and won't blow up when you have millions of samples.
If you're already on Prometheus + Grafana, use a histogram metric and let Prometheus calculate percentiles server-side with histogram_quantile(0.99, ...). That's the setup we run at Cubet; the moment P99 diverges from P50 on a critical endpoint, an alert fires before users reach support.
The Database Is Usually the Culprit
In most backend services, the tail traces back to the database. A query that averages 5ms occasionally takes 400ms. The usual suspects:
- Long-running transactions holding row locks while other queries queue
- Autovacuum or index rebuilds running concurrently with reads
- Hot rows — a single record (global counter, "last active" timestamp) written by many concurrent requests, creating write contention
PostgreSQL gives you a direct view into this:

A tail_ratio over 10 means your worst-case execution time is 10× your average. That query is creating your tail. stddev_exec_time tells you how erratic it is — high standard deviation is a sign of lock contention or resource exhaustion rather than a consistently slow query.
Strategies That Actually Help
Hedged requests — For latency-sensitive reads hitting external services, fire the same request to two backends simultaneously and take whichever answers first. Cancel the slower one. You spend a bit more on load; you buy meaningfully better P99. Google calls this "backup requests" and uses it pervasively. It's extreme medicine, but for your most critical endpoints it's effective.
Aggressive timeouts everywhere — Every downstream call — database, cache, external API — should have an explicit timeout set at your acceptable P99 ceiling. Don't let one 5-second database stall cascade through your whole request stack. Fail fast at the boundary, return a degraded response, and log it.
Connection pool right-sizing — There's a sweet spot between "too small, requests queue" and "too large, database drowns in connections." Monitor p99_wait_time from your pool metrics (most pool libraries expose this). If requests are regularly waiting for a slot, your pool is too small. If your DB CPU is spiking without load, your pool is too large.
Distributed tracing — Add OpenTelemetry spans to your service and its downstream calls. When a P99 request shows up in your slow query log or APM tool, you can see the exact breakdown: 2ms in the handler, 380ms waiting for a database connection, 10ms query execution. Guessing at root cause is expensive; traces make it obvious.
GC tuning in Node.js — Give V8 more heap headroom (--max-old-space-size=512 or higher) to reduce the frequency of major GCs. Fewer major GCs = fewer event loop pauses = a cleaner P99. You can also experiment with triggering GC during known-idle windows, though that gets into sharp-edge territory quickly.
The Mental Model That Changed Everything
Here's the shift that made the biggest difference in how I think about performance work: stop optimising for the happy path; start defining the worst acceptable case.
Pick a number. Is P99 under 200ms acceptable for your API? 500ms? Write it down as an SLO. Instrument it. Alert on it. The moment you define it concretely, you stop treating tail latency as an abstract concept and start treating it as a production commitment.
Averages feel good because they smooth the ugly parts. Percentiles show you what your users actually experience — especially the 1% who are one bad request away from closing the tab and not coming back.
Your P99 is your real boss. The average is just the PR spin.
If you're not currently tracking P99 on your critical endpoints, that's your one action item: add a percentile histogram to your most trafficked route and let it run overnight. Whatever comes out the other side — that's a conversation worth having with your team.
